Pitfall in Differentiation of Hemorrhagic Vs. Fatty Lesions in Female Pelvis Using Fat Saturated Sequences With Inversion Recovery – Role of T1 FATSAT Sequence – A Case Report with Radiological Review

چکیده
T1 hyperintense structure in female pelvis can be either a hemorrhagic lesion or fat containing lesion. FATSAT technique is integral part of workup of any T1 hyperintense structures in female pelvis for differentiation of hemorrhagic lesion and fat containing mature ovarian teratoma for tissue characterization. We present a case of hematometra with blood in bilateral fallopian tubes due to cervical stenosis. The hemorrhagic content appear hyperintense on both T1WI and T2WI. An unexpected signal decrease in FATSAT T2WI inversion recovery sequence (STIR) was encountered. Suppression of signal in tissue with similar T1 relaxation time as fat can lead to diagnostic pitfall in T1WI and T2WI STIR pulse sequence. Also loss of signal on T2WI can be due to T2 “shading” in T1 bright ovarian endometrioma. Hence, fat specific spectral fat saturation on T1WI (FATSAT T1) is strongly recommended for tissue characterization in gynaecological diseases. This sequence is useful in cases of ovarian endometrioma and hematometra.

The effect of inversion times on the minimum signal intensity of the contrast agent concentration using inversion recovery t1-weighted fast imaging sequence
Background :Inversion recovery (IR) pulse sequences can generate T1-weighted images with a different range of inversion time (TI) to suppress or null the signal intensity (SI) for a specified tissue. In this study, we aimed to investigate the effect of TI values on the concentration of the contrast agent, which leads to a minimum signal intensity, using an inversion recovery T1-weighted 3-dim...
متن کاملMR evaluation of vertebral metastases: T1-weighted, short-inversion-time inversion recovery, fast spin-echo, and inversion-recovery fast spin-echo sequences.
PURPOSE To compare the detectability of vertebral metastatic disease on T1-weighted, short-inversion-time inversion recovery (STIR), fast spin-echo (FSE), fat-saturated FSE, and inversion recovery FSE (IRFSE) MR sequences using percent contrast and contrast-to-noise ratios. METHODS Patients with proved metastatic disease underwent imaging on a 1.5-T MR system with sagittal T1-weighted (800/20...
متن کاملSarcomatoid Carcinoma of Renal Pelvis Involving Ureter and Renal Parenchyma with Heterologous Osteosarcomatous Differentiation: A Case Report and Review of Literature
Sarcomatoid carcinoma is a high‑grade rare malignant tumor with both epithelial and mesenchymal components. Sarcomatoid carcinoma in the upper urinary tract is very rare. We reported here a case of sarcomatoid carcinoma of renal pelvis with osteosarcomatous differentiation, with involvement of the ureter and renal parenchyma in a 68-year-old female. Histologically, predominant pleomorphic spind...

Lipoma arborescens, also known as villous lipomatous transformation of synovium is an unusual fatty lesion of joint. This disorder is characterized by a diffuse increase in the quantity of subsynovial fat in supra-patellar region which bulges the overlying synovial lining and produces a villous architect...
متن کاملI-45: Advance MRI Sequences in Pelvic Endometriosis
Background: To assess MRI in diagnosing endometriotic lesions, emphasizing T2*weighted imaging efficacy. Materials and Methods: This prospective study of 48 females (22-38 years, average 29.6) clinically suspected of endometriosis from September 2009 to April 2012. MRI was performed with a 1.5 T imager (Siemens) with a body array coil. T1, T2 and T2* weighted (2D-FLASH) sequences were obtained ...
